New Patient Registration

glasses_and_penWe always welcome registrations from anyone living within the practice boundary, which extends south from Wimborne to Merley and Corfe Mullen, north to Witchampton and Hinton Martell, west to Sturminster Marshall and east to Colehill, Holt and the borders of Ferndown (see map on next tab).

Patient rights and responsibilities

  • To receive a practice leaflet
  • To be treated with courtesy and respect
  • To be offered appointments within 48 hours with a healthcare professional
  • To be given information about your health and any treatment by a healthcare professional and have the opportunity to ask any worries or concerns questions or discuss
  • To receive a prescription within 48 hours of ordering
  • To attend promptly for appointments or to cancel appointments in good time if you cannot attend
  • To be removed from the practice list if violent or abusive to any of the healthcare team

Your right to confidentiality will be maintained

Online Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.  Please also complete and send the online medical questionnaire for new patients which is also below.
